In order for us to get to know you better, please complete the new patient paperwork and bring it with you to your first visit. It is important that this paperwork be as complete and accurate as possible. We look forward to meeting you. 

In addition, below is a checklist of information to bring for your first visit:

  • A list of current medications with dosage and frequency

  • A list of specialist physicians you are currently seeing

  • Insurance card and a valid ID

  • Copy of previous medical records, if applicable

​​Are you taking new patients?

Yes. We are accepting new patients at our Jog Road location and Federal Highway location. Please contact the office to schedule your new patient appointment.

How do I request a refill of a medication?

All prescriptions and refills for chronic medications can be requested by contacting the office during normal business hours. If it is a new medication, or for an acute issue, we ask that you schedule an appointment with one of our medical providers. Please call the office at 561.733.9690 (Jog Road) or 561.733.8600 (Federal Highway) and leave a message on our refill request line. Please allow 24 hours for all requests to be processed. You may also request refills by contacting your preferred pharmacy. 



How do I contact the office after hours?

​After hours, our answering service can reach the provider on call. We ask that for all non-urgent issues to please contact the office during normal business hours. 



What do I do in the event of an emergency?

In case of emergency please call 911 or go to your nearest emergency room.
